Online Check-In and Waivers
Approved users are asked to sign in at our main gate on the day of each visit. We have now simplified this process, and you may check in via an online form on a computer or smartphone. For frequent users, we suggest bookmarking the form url on your phone.
This is not a substitute for reserve use approval through rams.ucnrs.org.

Every Non-UC student and faculty Visitor to UCSC FONR MUST complete the following Covid symptom check and waiver (UC affiliates are also doing similar checks through a different system).
Every Day you visit, complete the following before entry:
When asked about the location you will visit, reply with:
Location: “MBEST Center”
Region: “Uncertain, Variable, Not Listed”
Building: N/A
For the primary contact list “Joe Miller” and joemiller@ucsc.edu, you will not need a secondary.
Once every 6 Months:
We have also made a new digital reserve use waiver, please share this link with your classes and groups before a visit. All users must sign a waiver before entry, and they are good for 6 months.

This waiver applies to adults only. For minor visitors who are approved for a reserve field trip, as with k-12 groups, we will still be sending individual documents home for parents to sign.
